Community Hospice was founded in 1999 and is a family-owned hospice organization serving patients and families across Southeast Georgia. For more than 20 years, the organization has been led by Jason Colbert, RN, CHPCA, who oversees all aspects of hospice care and operations. Community Hospice provides a full range of services including in-home hospice care, a 12-bed inpatient Hospice House located in Vidalia, Georgia, respite care, and General Inpatient (GIP) hospice services at Memorial Health Meadows Hospital in Vidalia. Community Hospice cares for patients of all ages, providing hospice services for both pediatric and adult patients with serious, life-limiting illnesses. Through the support of the Community Hospice Foundation, a 501(c)(3) charitable organization, additional resources are available to assist patients and families facing financial or personal hardships during the end-of-life journey.Today, Community Hospice proudly serves 14 counties across rural Southeast Georgia, providing compassionate, patient-centered care focused on dignity, comfort, and support for both patients and the families who love them.
